You can use the form below to report any violation, incident, or concern related to something you have experienced or witnessed.

Your contact details will be treated as strictly confidential. Reports are received by a dedicated team that does not include individuals directly involved in the Foundation’s day-to-day operations. At the initial stage, only selected staff members based in the Warsaw office have access to submitted reports.

Each report is handled in line with the Foundation’s established reporting and complaint procedures. More information about these procedures can be found here: Documents governing the operations of the PCPM Foundation.

Our top priority is to ensure the safety and protection of every whistleblower. Your personal information will not be shared with any person mentioned in the report unless you explicitly agree to it.
